Making A Referral
We work closely with families and health and social care professionals to support children and young people with life‑limiting and life‑threatening conditions.
Our referral criteria are designed to ensure that those who need specialist palliative care, respite, and family support can access our services at the right time.
Children and young people who meet one or more of the groups outlined below may be eligible for referral. Each referral is considered on an individual basis, and we welcome discussions at any stage of a child’s illness, including alongside active or curative treatment.
If you are unsure whether a child or young person meets the criteria, or would like to talk through a potential referral, please do not hesitate to contact us for advice.
Our Referral Criteria
Group 1
Life threatening conditions where curative treatment may be possible but can be unsuccessful. In these circumstances, access to palliative care services may be required. Examples include cancer and irreversible organ failure affecting the heart, liver, or kidneys.
Group 2
Conditions where premature death is unavoidable, but where there may be prolonged periods of intensive treatment aimed at extending life and enabling the child or young person to participate in everyday activities.
Group 3
Progressive conditions for which there are no curative treatment options. Care is exclusively palliative and may be needed over many years. Examples include Batten’s disease, mucopolysaccharidosis, and muscular dystrophy.
Group 4
Irreversible, non progressive conditions resulting in severe disability, leading to increased vulnerability to health complications and a likelihood of premature death. Examples include severe cerebral palsy or multiple disabilities arising from brain or spinal cord injury.
