Advance Care Planning (ACP)
A session for all healthcare professionals to explore how an Advance Care Plan (ACP) can improve end-of-life care.

At the end of the course I will be able to:
Understand and respect a person's preferences if they become unable to communicate for themselves.
Understand how Advance Care Planning can reduce a families' anxiety, depression and stress and cause them to be more satisfied with the care received.
Understand how it can reduce non-beneficial transfers to acute care and unwanted interventions.
Understand the importance of communicating person's preferences to relevant professionals.
